After some measurements, the spacers are the correct length and all required parts are there.

When the axle is tightened down, the forks, axle, bearings, and spacers become a solid "assembly" if you will, and the wheel hub is able to move side to side over this assembly... which indicates to me that the bearings are not seated properly in the hub or the part of the hub that holds the bearing is worn.

I've ordered new bearings and Loctite 640 High Strength Sleeve Retainer to help hold the bearings in their correct position.

Hopefully this solves the problem, I will update this post after the weekend.

New bearings with loctite did the trick, everything works as it should now.
